How to Fax Form 3911 and Trace a Missing Tax Refund

When the IRS asks for Form 3911, here is how to complete it, find the correct state-specific fax number, and keep proof of your refund trace request.

Form 3911 asks the IRS to trace a refund that was issued but did not reach you, was lost, or may have been deposited incorrectly. Before filling it out, check Where’s My Refund? and read any letter the IRS sent. Many current-year refund questions can be handled without a fax.

Use Form 3911 when the IRS directs you to it or when the official refund guidance says a trace is appropriate for your situation.

Complete one form for each refund

Use the current Form 3911. Identify the tax year, refund amount, type of return, and whether you expected a check or direct deposit. Add your current address and the address used on the return if they differ.

For a jointly filed return, both spouses generally sign the request. Prepare a separate Form 3911 for each refund you need the IRS to trace.

Find the correct Form 3911 fax number

The IRS publishes a state-by-state fax table on the Form 3911 information page. Use the number listed for the state where you currently live, not the state where you lived when you filed the return, unless a letter from the IRS tells you to use a different destination.

Do not send unrelated documents to those numbers. The IRS specifically identifies them for Form 3911 refund trace requests.

How to send the refund trace

  1. Complete the form using the information from the relevant tax return.
  2. Sign it in every required place.
  3. Scan the full form as a clear PDF.
  4. Enter the state-specific number from IRS.gov or the number in your letter.
  5. Preview the pages, send once, and save the confirmation.

Avoid sending duplicate copies by fax and mail at the same time. If you need to follow up, keep the date, destination number, and delivery report ready.

What a fax confirmation does and does not prove

A successful report shows that the receiving fax system answered and accepted the transmission. It does not mean the IRS has reviewed the form or approved a replacement refund.
