Free Fax Cover Sheet Template: PDF and Word

Download a clean fax cover sheet or copy the simple format. It includes the recipient, sender, page count, subject, and a short message.

A fax cover sheet tells the receiving office who the fax is for, who sent it, and how many pages should have arrived. It should be short. One page is enough.

Use the PDF when you want to print and write by hand. Use the Word version when you want to type the details before saving or faxing.

What to put on a fax cover sheet

  • To: The intended person’s full name.
  • Fax number: The number you are sending to.
  • From: Your name or organization.
  • Your contact: A phone number or email for questions.
  • Date: The date you send the fax.
  • Total pages: Include the cover sheet in the count.
  • Subject or reference: A short case, claim, invoice, or account reference.
  • Message: One or two sentences explaining what is attached.

If the office gave you a case or reference number, include it exactly as written. That is usually more helpful than a long note.

Does the cover sheet count as a page?

Usually, yes. If you send a three-page form plus one cover sheet, write four total pages. Online fax services may also count that cover sheet toward a free or paid page allowance.

Some apps generate the cover page separately. Preview the final transmission so you know whether it has been included and where it appears.

Keep private details out of the message

A cover sheet may be seen by reception staff or anyone monitoring a shared fax inbox. Include enough information to route the document, but avoid copying sensitive medical, financial, identity, or legal details into the message.

For sensitive material, follow the recipient’s instructions. A generic confidentiality sentence does not replace sending to the correct number or using an approved process.

Do you always need a cover sheet?

Not always. If you are replying through an app to a known contact and the document already identifies you clearly, the recipient may not need one. Government offices, clinics, insurers, and shared business fax lines often benefit from a cover page because it helps staff route the pages.

When an organization provides its own cover sheet, use that version. It may contain barcodes, reference fields, or routing information that its system expects.

Open the Word template, fill in the fields, and save it as a PDF. Place it before the main document in your fax app. Then check that the total page count matches and every page is readable.
