How to Fax Form 4506-T to Request a Tax Transcript

How to prepare Form 4506-T, find the correct IRS fax number, and decide whether the online Get Transcript service is a better option.

Form 4506-T requests a transcript of a tax return or other tax account information. A lender may ask for it during a mortgage or business application, but you can also request your own transcript.

Start with the IRS Get Transcript service. If it provides the record you need, the online route is usually faster than faxing a paper form.

Choose the right transcript

Form 4506-T can request several records, including a return transcript, account transcript, record of account, wage and income transcript, or verification of nonfiling. Read the descriptions on the form and select only what the recipient actually needs.

Enter the current name and address, then the name and address shown on the tax return if they were different. List the tax form number and up to four ending dates or years.

Sign it within the allowed time

The current form requires a signature and date, and the signer must check the authority box above the signature area. The IRS must receive the request within 120 days of the signature date.

For joint returns, either spouse can generally request many transcripts, but read the form instructions for the record and delivery method involved.

Find the Form 4506-T fax number

The destination depends on where you lived or where the business was located when the return was filed. Use the current IRS Form 4506-T filing table, which may be updated separately from a printed copy of the form.

Do not use a fax number from an old blog post, saved PDF, or previous application. Check the IRS page on the day you send.

Send and retain a clean record

  1. Confirm the transcript type and tax years.
  2. Complete the identity and prior-address fields.
  3. Check the authority box, sign, and date.
  4. Scan the whole form with no cropped margins.
  5. Use the current IRS location table.
  6. Save the PDF and successful transmission report.

Be cautious if a third party asks you to direct the transcript to an unfamiliar address. Verify the organization and understand what information the transcript will reveal before authorizing disclosure.
